ELAR Depositor Forms

There are two versions of the Deposit Form:

Both forms have the same content. You can download and print out (1) to fill in as paper form. However, we recommend that you print out (1) and use it as a reference but fill in (2), which will provide you with an electronic copy that you can both print for your records and email to us.

In the future, we will also provide an on-line version of the form.
